CLINIC INFORMATION:

8:00 - 4:30, Monday , Tuesday, Thursday and Friday
8:00 - 4:00 Wednesday - all doctors are out on Wednesday afternoon,
but there is a nurse available.
Lab Hours:
8:00am - 12:00pm and 1:00pm - 3:00pm - Monday-Friday

PHYSICIAN INFORMATION:
Paul E. Valentin-Stone, M.D.
He has been in the Jacksonville area since 1983 were he was
assigned to the Air Force Base Hospital as Chairman of the
Dept. of Internal Medicine. He founded North Pulaski Internal
Medicine in 1985.
Ann D. Layton, M.D.
Dr. Layton joined the clinic in 2003. She graduated from Washington
State University and finished her residency at University
Medical Center where she specialized in Internal Medicine
and Geriatrics.
Clinic Staff:
Vanessa Hendershot - Office Manager
Vanessa has been with the clinic since 2002. She is in charge
of the daily operations of the clinic. She is the person to
contact for any concerns you may have regarding your visit
here. You may email her for any questions or concerns at slp1401@aol.com.
Nurses:
Linda Lowery - Dr. Stone's nurse
Monique Mosby - Dr. Layton's nurse
Linda & Monique will take you back to see your doctor.
They are available for questions regarding your lab work,
refills on medication and health questions. They will refer
your questions to your doctor, if necessary. They also assist
the doctor with any procedures that may be done in the clinic.

Sheryl will probably be the first one you see when you come
in for your appointment. She is responsible for updating any
insurance information, making sure you have read and understand
our Privacy Practices and collecting your copays and deductibles.
If you are a new patient, she will make sure all the proper
paperwork is filled out and get your chart ready for the nurse.
Amanda is also at the front desk. She will help you check
out and she assists Sheryl with the phone and entering your
information in the computer system. She keeps up with the
charges and payments for the clinic and she is available to
answer any questions you may have regarding any bills you
may receive from us. Carla works part-time and is available
to fill in at the front desk or for the nurses. Nicole will be in charge of setting up your appointments for other specialty doctors and putting in your referrals.
Patient Responsibilities:
If you are a new patient to our clinic, please arrive 30 minutes
prior to your appointment time, so that you have time to fill
out the paperwork. Please be sure to bring all of your insurance
cards. We will be scanning them in your chart. If you currently
take prescription medication, bring a list so the nurse and
your doctor know what you a taking. Once you are an established
patient, make sure that the front desk knows if there have
been any changes since your last appointment (new address,
phone, insurance, etc.).
If Dr. Stone or Dr. Layton is your Primary Care Provider and
your insurance requires a referral to see a Specialty Provider
(Tricare Prime, Health Advantage, Medicaid, etc.), please
let us know ahead of time, so we can get the referral to the
Specialist. According to your insurance policy, it is your
responsibility to make sure you have the referral before your
appointment with a Specialty Provider.
Other Information:
Prescription Refills:
Due to the tremendous amount of prescription refill requests,
please allow 48 hours for us to refill your medication. We
do not fill narcotic prescriptions on holidays or on the weekend.
Lab Results: A nurse will
not call you if your lab results come back normal. You will
be called if your lab results come back abnormal. We will
probably schedule a follow up visit with your doctor if this
is the case. If you have questions concerning your lab work,
you may call your nurse.
Appointments: If you will
not be able to keep your scheduled appointment, please call
us within 24 hours so that we may offer that appointment time
to another patient.
It is our goal to provide you with outstanding medical care.
We feel our doctors are all wonderful and caring people. We
have an awesome staff that is willing to help you however
we can and we look forward to meeting your medical needs!
